   2. Download, print, fill in and sign the Waiver Form.
   3. Submit your Membership Application and Waiver in one of the following ways:

   4. Submit your payment in one of the following ways:

• by mail together with your completed forms. Make cheque payable to Wild Bettys.
• on line by using PayPal or major credit card via PayPal
• interac email transfer to melinda@wildbettys.com

Once we recieve all the forms and payment you will gain access to the member's forums on this website. You will recieve an official OCA/Wildbettys membership card which will entitle you to all the great deals and specials Wild Bettys members enjoy.

All information will be kept private & confidential to the Wild Bettys.
